Administrative matters of stay in the ČR

Reporting changes of an EU citizen

All foreigners are required to report to the Ministry of Interior ( MOI – OAMP) all changes during their stay in the Czech Republic.

Then, please contact your Mobility Coordinator to book an appointment at the Ministry ( MOI – OAMP).

Change of address

Change of your address has to be report at the MOI – OAMP within 30 days of it's occurring. This change is without any fee and you will get a stamp in your passport or you will get a new Certificate of Temporary residence. You will need these supporting documents:

  • Proof of accommodation at the new address
  • Travel document

Change of travel document/passport

Change of your travel document/passport has to be report at the MOI – OAMP within 3 days of its occurring. You will need to bring your new passport with you. This change is without any fee.

Change of your name/surname

Change of your name or surname has to be reported at the MOI-OAMP within 3 days of its occurring. You will need your new passport or identity card.

The fee for changing your name or surname is 1000 CZK in a form of revenue stamp (kolek). 

Change of marital status

Change of your marital status has to be report at the MOI-OAMP within 3 days of its occurring. This change is without fee. You will need supporting documents:

  • A document confirming the change of marital status (ex.: marriage certificate etc…)
  • Travel document/passport 

If this change also involves the change of your surname you will have to provide with a new passport with the new surname. 

Other changes

All other changes have to be reported to MOI – OAMP within 3 days of their occurring. You will need a document confirming the change and a travel document/passport.

Change of address

Change of your address has to be report at the MOI – OAMP within 30 days of its occurring. If you are a  holder of a long-term residence permit, this change is without any fee and a new card is not issued. In this case, you are not oblidged to visit the MOI in person and you can  send the documents via post.

The below supporting documents are required:

  • Proof of accommodation at the new address
  • Travel document

If you are a holder of a permanent residence permit, a new card will be issued, therefore you will be charged an administrative fee equal to 1000 CZK and you are oblidged to visit the MOI – OAMP in person to provide your biometric data.

Your Mobility Coordinator will book an appointment for you at the MOI-OAMP.

Change of travel document/passport

Change of your travel document/passport has to be report at the MOI – OAMP within 3 days of its occurring. You will need to bring your new passport with you. This change is without any fee.

Change of your name/surname

Change of your name or surname has to be reported at the MOI-OAMP within 3 days of its occurring. You will need your new passport or identity card.

A child born in the Czech Republic

If your child is born in the territory of the CR during your stay on a long-term visa, your are required to file an application on behalf of your newly born child for a long-term visa at the MOI-OAMP within 60 days from the day of birth. The following documents are required:

  • Travel document/passport (of the legal representative)
  • Green application form (purpose DÍTĚ NAROZENÉ NA ÚZEMÍ ČR)
  • The child’s birth certificate
  • Medical insurance for the child

The administrative fee is 1000 Kč (kolek)

Other changes

All of other changes has to be reported to MOI – OAMP within 3 days of their occurring. You will need a document confirming the change and a travel document/passport.

If you lose your travel document/proof of identity/residence card or wallet, you are obliged to notify the Czech police (Policie České republiky) within 24 hours. At the police station you will fill-in a report. The police is obliged by law to provide you with an interpreter, however, there is not always an English speaking person to help you immediatelly.

We recommend you to go to Malá Strana police station or  Police station at Jungmannovo náměstí. They have English speaking officers.
